To be eligible you must have a current exempt status under Section 501(c)(3) of the internal revenue code or be a public school. THIS FORM MUST BE FILLED OUT IN ITS ENTIRETY AT LEAST 4 WEEKS PRIOR TO YOUR EVENT OR YOUR EVENT DEADLINE TO BE CONSIDERED.
